Background Check Forms:
In order to provide a safe environment for all players in Monarch Little League all volunteers including new and returning managers, coaches, assistants, team parents, board of directors, and umpires are required to complete a background check prior to the start of a season.
At the beginning of each playing season (once the teams have been formed) the League President will send out an email to all volunteers which will include an online form to be completed. The online form is much simpler to complete than the print forms listed below.
